Test request form
Every sample must arrive with a completed Test Request Form (WWT/LQMS/051). It asks for:
- Customer name and address, and the project the test is for
- Contact person, mobile number and email
- Description and number of samples
- Tests requested, with the standard or customer requirement
- Whether you want the sample returned after testing
- Names of any witnessing persons
- Report dispatch mode: courier or hand delivery
Storage and disposal
Tell us on the form whether you will take the sample back. Samples not collected within 15 days of testing are automatically scrapped.
Handling
The lab takes every precaution while handling your samples, but cannot take responsibility for damage or loss before a sample is received.
Collection guidelines
Water
- Use a clean, sterile, leak-proof bottle. Rinse it with the sample water before filling unless it is a sterile microbiology bottle.
- Let tap water run for 2–3 minutes before collecting.
- Label each bottle with the source, location, date and time of collection.
Wastewater
- Collect a representative sample from a well-mixed point, avoiding surface scum and settled sludge.
- Keep samples cool and deliver them as soon as possible. BOD and COD samples are time-sensitive.
- Note the outlet or treatment stage and whether the sample is grab or composite.
Microbiology
- Use only sterile bottles. Do not rinse them, and do not touch the inside of the cap or bottle.
- Leave air space at the top of the bottle.
- Keep samples chilled and deliver within 24 hours of collection.
Food & Dairy
- Submit products in their original sealed packaging where possible.
- For loose items, use clean, sealed food-grade containers.
- Keep perishable and dairy samples chilled during transport.

Your test report
Reports for accredited tests carry the NABL mark. Each one lists the parameter, method, result and unit, with the acceptable and permissible limits where a standard applies, such as IS 10500:2012 for drinking water.
Every report is signed by the technical manager and sent by courier or hand delivery.
